What is Troponin?

Troponin is a term used to describe three distinct proteins. Troponin C binds to calcium and delivers troponin I to muscles, allowing them to contract. T Type Troponin is a protein that interacts with troponin proteins in muscle fibers. The Troponin test is used to check for heart attack by measuring troponin I or troponin T levels in the blood.

The heart is essentially a muscle, and when it is damaged, troponin is released into the bloodstream. Troponin levels in the blood are generally quite low, but cardiac damage can cause them to spike rapidly.

What is Troponin Test meant for?

A troponin test determines how much troponin is present in your blood. Troponin is a type of protein found in your heart’s muscles. It is a protein that is not regularly shown in the bloodstream. Troponin is released into the bloodstream when cardiac muscles are injured. As cardiac damage worsens, more troponin is released into the bloodstream.

Troponin levels in the blood may indicate that you are experiencing or recently had a heart attack. When the blood supply to the heart is blocked, a heart attack occurs. This blockage has the potential to be fatal. However, early detection and treatment can save your life.

The most common use of this test is to diagnose a heart attack. It is sometimes used to track angina, a disorder that causes chest pain by restricting blood flow to the heart. Angina can lead to a heart attack in some cases.

This test may also be used after you’ve been admitted to the hospital after being diagnosed with a heart attack. In most cases, testing is performed twice or more in a 24-hour period. This is done to check if the levels of troponin had fluctuated over time.

In addition to detecting your troponin levels and monitoring your ECG or EKG, your doctor may recommend further tests to assess your health, such as:

What is a Normal Troponin Level?

A troponin normal range level is one that falls within the usual reference range. So, if a lab says < 0.4ng/l  is normal, then 0.3, 0.2, 0.1ng/l, and so on are all normal.

When a high-sensitivity cardiac troponin T test finds levels greater than 14 ng/l, heart damage or a heart attack is likely to happen. 

Troponin levels beyond a certain threshold are an urgent warning sign. The more troponin — specifically TnI and TnT — released into the bloodstream, the more troponin is released into the bloodstream, and the greater the risk of cardiac injury. Troponin levels can spike within 3 to 6 hours after the heart has been injured and stay high for 10 to 14 days. 

Though a spike in troponin levels is frequently associated with a heart attack, your troponin levels could be elevated for a variety of causes.

Troponin levels can be either abruptly or chronically high. If you have symptoms in addition to a shift in troponin levels, your symptoms will be used to help a healthcare professional make a diagnosis.

High troponin levels can be triggered a variety of heart problems, including:

  1. Cardiomyopathy
  2. Myocardial infraction
  3. Pericarditis
  4. Heart Failure

  6. Endocarditis

What Causes High Level of Troponin?

High troponin levels are commonly caused by damage to cardiac muscle cells. Other causes include kidney disease, stroke, infection, pneumonia, critical sickness, and a variety of others.

Troponin levels that are lower but still high could indicate a different diagnosis.

Elevated troponin levels can be caused by a variety of factors, including:

  3. Chronic kidney disease (CKD) or renal failure
  4. Failure of the heart
  5. Heart damage caused by chemotherapy
  6. Pulmonary Embolism

  8. Myocarditis is the inflammation of the heart muscle.
  9. Recreational drug use, such as cocaine, can cause heart damage
  10. Catastrophic cardiac damage, such as one caused by a sudden, strong hit to the chest

How to Reduce Troponin Level?

If only a small amount of troponin is present in the blood, it usually indicates that the heart has been damaged, but if it doesn’t rise and soon falls, it’s most likely due to a health problem other than a heart attack/cardiac arrest. Treatment may not be essential in this scenario, although it depends on the individual.

To reduce troponin levels, the underlying cause of the elevated levels must be addressed. It’s also a good idea to enhance your cardiovascular health in general, such as by lowering your blood pressure or cholesterol.

According to certain studies, statins can assist to decrease elevated troponin levels.

Patients with high troponin levels, when administered with Statin, had a fivefold lower risk of heart attack or death from coronary heart disease than those whose troponin levels were unaltered or raised.

In those who are at high risk for cardiac arrest, statins are used to prevent coronary heart disease. “A drop in troponin could indicate treatment is beneficial, whereas any increases in blood troponin could trigger a change in the treatment plan.
